Care isn't one event — it's a sequence. The Phase Framework is our way of mapping each stage of care to the routines and equipment that fit it, so families face one manageable step at a time instead of everything at once.

Why phases, not categories

Walk into most home-health information and you'll find a wall of products and conditions, sorted by category. That's fine if you already know exactly what you need. But families rarely do. They know their situation — "Mom keeps the house but the bathroom worries me," or "Dad comes home from hip surgery on Thursday" — not a product aisle.

The Phase Framework starts from the situation. It recognises that needs follow a predictable arc, and that the most helpful thing isn't a longer list of options — it's knowing which options matter right now, and which can wait. Match the help to the moment, and the overwhelm drops away.

The phases of care

Most people pass through one or more of these. They aren't rigid stages — they overlap and shift — but naming them is the fastest way to find your footing.

Aging in Place

The senior phase: staying safe and independent at home as mobility and balance gradually change. Priorities are fall prevention, bathroom safety, and gentle home modifications that keep daily life manageable.

Recovery — Phase 1: Acute

The first days and weeks after surgery or injury. The goal is rest, protection, and getting the essentials set up so the basics of daily life are safe and reachable.

Recovery — Phase 2: Regaining Mobility

Healing turns into moving again — stepping down from heavier equipment to lighter aids, gentle range of motion, and rebuilding everyday confidence.

Recovery — Phase 3: Strength & Conditioning

Rebuilding strength and stamina and returning to normal activities safely, supported by simple conditioning gear rather than recovery equipment.

Chronic Care

Living well with a long-term condition. The focus shifts from a single recovery to steady routines, repeat-need supplies, and planning ahead so nothing runs short.

Home Hospital

Advanced care delivered at home. The work here is turning a room into a safe, capable care space — beds, lifts, pressure relief — with dignity and the caregiver's wellbeing in mind.

How matching to your phase helps

When equipment and routines are matched to the current phase, three good things happen:

  • Less is wasted. You buy or borrow what fits now, and step up or down as the phase changes — instead of guessing at everything up front.
  • Safety improves. The right aid at the right time prevents the falls and strains that come from using equipment that no longer suits the person — or skipping it too soon.
  • The decisions feel smaller. One phase at a time is something a tired caregiver can actually handle. The whole journey at once is not.

The framework is a way of thinking, not a prescription. Your care team knows your specifics; we just help you see the shape of the road ahead.
